Antique Consignment Intake & Condition Packet Builder

Create a clear intake packet for a single antique or a mixed collection. Capture who delivered each object, what was observed, what is only claimed, how it arrived, and what must be clarified before a listing or hand-off.

Owner & intake details

Record the people and event connected to this intake. Avoid adding sensitive information you do not need.

Items & observed condition

Use a separate entry for each object. Describe visible facts and exact locations; keep a label or owner statement in the claim field.

Good condition note: “Front-left corner has a 2 cm loss with pale wood visible” is more useful than “fair condition.” Do not clean, test, dismantle, or diagnose an object as part of this worksheet.

Attribution, price & sale instructions

Capture what was represented and the figures provided. These fields do not calculate value or determine an appropriate reserve.

Custody & receipt checklist

Mark completed steps and note exceptions. A check records workflow status; it is not proof of legal ownership or title.

Disclosures & questions to resolve

Record statements in neutral language and track follow-up before cataloging, display, or sale.

Antique consignment intake: common questions

What should an antique consignment intake form include?

Useful records commonly identify the owner and receiving party, each object, intake date and location, observable condition, stated attribution, price instructions, disclosed repairs or damage, receipt details, custody changes, and follow-up questions. Adapt the fields to your written agreement and workflow.

How do I document antique condition without overclaiming?

Describe visible characteristics, their location, and approximate extent. Separate those observations from the consignor’s statements and from any specialist’s opinion. Avoid unsupported words such as “original,” “untouched,” or “authentic.”

Should an owner’s attribution be recorded as fact?

No. Preserve the exact wording or a neutral summary, identify who supplied it, and note any supporting records separately. An owner statement is a claim or lead until independently assessed.

Does the reserve field recommend a price?

No. It only records an amount or instruction supplied for the intake record. It does not estimate fair market value, advise on a reserve, or calculate sale proceeds.

Where are my intake details saved?

The tool builds the packet in your browser and does not send the entries to this site. Export JSON or print a copy if you need to retain it. Be mindful of personal data on a shared device.

Consignment intake packet workflow

Start with the consignor and receipt context, then create an individual record for every object rather than combining a group into one ambiguous description. Capture a neutral inventory name, object identifier, arrival condition, dimensions or marks worth recording, and a brief note about the location of visible wear or damage. If a claim about maker, age, origin, or ownership is supplied, identify its source and keep it distinct from what the receiving person observed.

Use the custody checklist to note item count, tagging, photographs, storage location, and hand-off exceptions. Price and reserve fields are for recording instructions and their source only. Before catalog publication or transfer, review unresolved title or authority statements, condition disclosures, owner permissions, repair history, and specialist questions under your own procedures.

Entries stay in the current browser session until you clear the form or leave the page. This tool is not a substitute for a signed consignment agreement, legal review, professional appraisal, conservation examination, or authentication opinion.
